Medical statements
To request medical statements from our staff, a patient consent form (Police request CID68) must be completed by the police, local authority or law enforcement agency and signed by the patient. A MG11 witness testimony form must be also completed with the medical statement by an appropriate clinician. This should then be submitted via email to: cdda-tr.
Medical notes and patient related documentation
To request medical notes (e.g. x-rays, scans, emergency department records or any other patient medical documentation), a signed patient consent form (Police request CID68) is required. If it is not possible to receive a patient consent form, a NPCC TRM Request Form must be submitted. This must be completed by the police, local authority or law enforcement agency fully stating what information is required, alongside the reason the information is required and why it is being requested. It must be signed by a police sergeant or above or a senior manager in other agencies and is subject to verification of that person's status. This should then be submitted via email to: cdda-tr.
