Feel better awardNHS 2010 Awards 21Who's made you "feel better" when receiving care from us?

"Coming to hospital, or receiving care in your own home or one of our community bases can be a difficult time, so we'd like to know who among our staff has made you "feel better" about your treatment.

"It may have been one of our hospital staff, or staff who have helped you at home or in the community. It may have been one of our support staff - a cheerful porter or a helpful receptionist perhaps.

"The Trust holds an annual staff awards ceremony to highlight good practice and excellent quality care/service.  This year, we would like patients, visitors or carers to get involved and have their say.

"You can nominate one of our staff for the making you "feel better" award by filling in this form (see below)
and returning it to us.  The closing date for the award is Friday 23rd September 2011. 

"If your nominee wins you will be invited to attend the  Annual Awards Ceremony and celebrate the achievements of our staff on 25th October 2011, Hardwick Hall, Sedgefield, Co. Durham.

"We look forward to hearing from you."
