Police requests for information held by County Durham & Darlington NHS Foundation Trust
Medical Statements.
To request medical statements from Trust staff a patient Consent Form, 'Police request CID68' must be completed by the Police/Local authority or Law enforcement agency and signed by the patient. A MG11 witness testimony form must be completed with the medical statement by the appropriate clinician. This should then be submitted to the Police request inbox: cdda-tr.Policerequests@nhs.net
Medical notes and patient related documentation.
To request medical notes /X-rays/Scans/A&E records or any other patient medical documentation from the Trust, a signed patient consent form, 'Police request CID68' is required. If it is not possible to receive a patient consent form, a DP2 form must be submitted. This must be completed by the Police/Local authority or Law enforcement agency fully stating what information is required, the reason the information is required and why it is being requested. It must be signed by a Police Sgt or above or a senior manager in other agencies and is subject to verification of that person's status to cdda-tr.Policerequests@nhs.net
