Eating enough to help patients meet their nutritional needs at home

When patients initially go home they may experience a poor appetite. To help meet their nutritional needs it is important to adapt their food and fluid to increase the calorie and protein content, to make sure every mouthful they take is full of nourishment. Try to include the drinks and snacks the patient may have received in hospital, such as:

  • Eat two nourishing snacks each day
  • Drinking nourishing drinks, such as milky coffee, hot chocolate/malted drinks made with milk, fruit juice
  • Drinking 1-2 fortified drinks each day (recipes provided in the leaflet below)
  • Fortifying certain food dishes, such as soup, custard, milk pudding, porridge
  • Eating the fortified mousse (recipe provided in the leaflet below)
  • Using full fat products, such as milk, yoghurt, cheese, margarine, unless there is a medical reason.

Practical learning on how to enhance nutrition at home

Food First Workshops are delivered monthly at various venues, to provide patients and/or their family/carers with practical advice on how to adapt meals to ensure good nutrition continues at home.

Hot and frozen meal provision at home

Initially when patients go home they may not feel well enough to prepare meals. To support patients during this time, there are many organisations who deliver hot or frozen meals directly to the patient's home; as well as luncheon clubs and support services. For information on:


Monitoring of nutrition at home

  • It is recommended the patient continues to monitor their weight weekly once transferred home.
  • If the patient wants to monitor their risk of undernutrition, click here to visit the Focus on Undernutrition website which provides an online self-calculator.
  • If the patient has any concerns about their weight, they are recommended to contact their GP, who may refer them to a dietitian.
