The falls and osteoporosis services across County Durham and Darlington aim to help prevent falls and fractures, to provide support to people and help them to maintain active and independent lives. Falls Where falls are happening, our services will look at all the possible causes in order to prevent further falls occurring. We will assess the 'bone health' of clients in order to give them appropriate help and advice so they can avoid fractures. We encourage older people to lead lives giving advice on diet exercise and safety in the home. Our service includes specialist nurses, occupational therapists and physiotherapists. We have close links with other services including GPs. If you have had a fall, feel you are at risk of falling, or are concerned about an older person you know, please contact the falls and osteoporosis services in your locality. The specialist nurses will assess patients needs and offer advice and treatment where needed with onward referral to other services where appropriate, the nurses work closely with rheumatologists and orthopaedic consultants and your GP to ensure that the best care is provided.
