Providing specialist diabetes care in the community. The community diabetes service is part of the County Durham & Darlington Community Health Services, specialist services directorate. The service covers the population of Derwentside and Durham & Chester Le Street. Our team consists of a specialist services manager, GPs with special interest (GPwSI) in diabetes (clinical lead), diabetes specialist nurses, dietitians and administrators. The services and all GPwSI's working within the community service have recently been through an accreditation process run by County Durham PCT and the Northern Deanery. The accreditation process monitors competencies, performance, up to date clinical practice and continuous professional development (CPD) of all staff. All services passed this process and are deemed experts within their field. The service is well established and was created to ensure that all people with diabetes Type 2 living in the Derwentside and Durham & Chester Le Street have equal access to high quality diabetes services which are closer to their homes and easily accessible. We provide a professional and efficient service to all the patients and include patients from all social, economic and ethnic backgrounds. We help residents in how to manage their diabetes by providing group education sessions, we can also help individual patients through one-to-one sessions with a specialist nurse/dietitian. We hold clinics across the community to provide easier access to specialist diabetes care. The community diabetes service also provides support for all the GP practices, running specialist clinics alongside normal practice teams to bring specialist care into local communities. The multi-disciplinary team is working closely with your GP practice and the diabetes unit at County Durham & Darlington Acute Hospitals NHS Trust to support people to manage their conditions more effectively.

They may have a foot examination, have their blood pressure taken and will be required to give a urine sample. Their medication and glycaemic control will be reviewed and discussed. All the cardiovascular risk factors will be investigated, reviewed and discussed.
