About our service
Our Macmillan Joining the Dots service supports people aged 18 and over who have been directly affected by cancer to live well during their cancer diagnosis, treatment and beyond. We also support the families, friends and carers of those directly affected by cancer. The help and support you receive is completely led by you and even if you think you don’t feel like you need our service right now, our team can still listen and make sure you have the right support when you do feel you need it.
We can support you with your emotional wellbeing, spiritual and religious needs, as well as finding the most appropriate services to support with practical issues such as welfare rights, benefits and transport.
This service was commissioned by Durham County Council and Macmillan Cancer Support and is delivered by the Adult Wellbeing Service which is a partnership between our Trust and the Pioneering Care Partnership (PCP).
How to access our services
If you would like support from our team, you can self-refer over the phone, via email, or have a face to face chat with one of our team members. You don’t have to be referred by a health professional like a doctor or nurse. However, with your consent, your doctor, nurse, social worker or other health professional can email or telephone our team on your behalf. One of our facilitators will then contact you directly to arrange an appointment to provide you with more information about how we can help and support.
