Prior to referral, please direct patients to our patient resources and leaflets.
There are several ways you can be seen by a Registered Dietitian. Please see the Dietetic Referral Criteria for information on how to refer to the dietitian.
If you are a patient, please ask a registered healthcare professional or GP to refer you to the dietitian. If you have a question about your appointment, such as changing or cancelling, please get in touch.

The resident should be high risk ('MUST' 2 or more) and already trialled food based treatments and powdered nutritional supplement. However, urgently refer the resident to the dietitian if recommended thickened fluids and is high risk of undernutrition and requires nutritional supplements. This is to ensure the resident is prescribed the safe consistency of nutritional supplements.
Please refer via the GP or Community Matron/Advanced Nurse Practitioner (ANP) using the Dietetic Referral Form. Please provide the refer with as much relevant information as possible, such as changes in weight, risk of undernutrition/'MUST', types of nutrition treatments already trialed, other medical conditions, such as dysphagia. Refer to the care home referral criteria.

We provide a service for patients who meet our referral criteria in the Durham and Chester-Le-Street, Durham Dales, Derwentside, Sedgefield and Darlington localities. If the child or young person's GP falls under the Easington locality, please refer to the North Tees and Hartlepool dietetics department. Please ensure all relevant information is included as stipulated in the referral document to avoid delay in processing and/or the referral being rejected due to insufficient information. We do not accept referrals for children's weight management (obesity), but Healthcare Professionals can refer patients into the Paediatric Weight Management Service via a separate referral form. They accept children with a BMI above the 99.6th centile, or above 98th centile with co-morbidities / aged under 2 years.
