As part of our outpatient rehabilitation provision, we provide a range of services and support, including:
We provide specialist assessment and advice to people who have suffered falls. If you have had a fall, we will look at all the possible causes in order to prevent further falls occurring. This may involve home safety advice, an exercise programme to improve your strength and balance or a medical assessment. We encourage and support older people to lead active and independent lives.
We provide support and advice to people leaving hospital after suffering a stroke. This includes providing community follow-up care, ongoing rehabilitation and home reviews. The team also provides multidisciplinary outpatient services for patients following a stroke and we provide assessments and treatment for a wide range of neurological conditions, including following a head injury, Multiple Sclerosis and Parkinson's.
We are a multi-disciplinary team who undertake assessment, treatment and rehabilitation for people with complex needs or who require support after leaving hospital.
We provide full Physiotherapy assessment and treatment for patients in their own home - including residential and nursing homes and community hospitals.
We provide functional assessments and rehabilitation programmes for patients in their own home and within community hospitals.
We provide a rehabilitation programme to those patients referred from either a rehab ward on one of our acute sites or following GP admission to hospital. This may be related to a variety of clinical concerns including orthopaedic, neurological, falls history or problems of the elderly. We aim to provide rehabilitation so you can regain as much independence as possible in order to return home safely.
