About our service

We are a multi-disciplinary team providing enhanced support for post critical care patients. The mirror image teams on both acute sites comprise of:

  • Nurse
  • Physiotherapist
  • Speech and language therapist
  • Dietitian
  • Counsellor
  • Associate practitioner
  • Critical care consultant

The team is coordinated jointly by the nurse and physiotherapist, aiming to ensure that patients have the service they need at each stage of their journey and particularly at transition points along the way. We hold weekly team meetings on each site. 

In patient support

Access to specialist speech and language therapy, dietetic and physiotherapy input. Enhanced support on the ward from associate practitioners working on behalf of all professions involved, delivering personal goal centered rehabilitation plans.

Outpatient follow up

  • Telephone support from team coordinators aiming to ensure all planned follow-up in place.
  • Dietetic follow up calls.
  • Access to counselling service specific to critical care trauma.
  • 3 month consultant led clinic. In line with guidelines all patients are offered a 3 month follow up clinic to discuss their care, ensure all other planned follow up actioned, pharmacy review and potential unit visit if desired.

The team engages in regional “life after critical illness” networks and seeks to engage with other services for out of area patients.

Accessing our services

 All critical care patients are screened at 72h and their needs assessed.
