About our service

Our Wellbeing for Life service offers a range of personalised support to people aged 16 and over who live and work in County Durham. We can advise and support you to start to make positive changes with your lifestyle and routines.

Our service focusses on basic weight management and healthy eating, low level mental wellbeing, stopping smoking and guidance on reducing the use of drugs or alcohol. We also help people to access other appropriate services to ensure that you are getting the right support. 

Our team will work with you for up to twelve sessions to set a health plan which will include small achievable goals that you would like to reach. We offer one to one sessions which can be face to face, over the phone or via video call, and we also offer group based sessions in community venues across County Durham. Wellbeing for Life also offers virtual and face to face educational workshops including sleep hygiene, mindfulness and building personal resilience to change.

Our Wellbeing for Life is a service commissioned by Durham County Council and delivered by our Trust (CDDFT) and Pioneering Care Partnership (PCP) who have years of knowledge and experience of working with and supporting local people across County Durham.

How to access our services

You can self-refer into our service by contacting us, or your GP, nurse, health care professional or any other health and social care worker can refer you to our service. We work from local community venues and workplaces across the County, so our dedicated teams are equipped with a wealth of local knowledge and keep up to date with what’s going on in your community.
